Medication review (MR) is one of the most important activities in daily clinical pharmacy
practice. Clinical pharmacists are able to detect drug-related problems (DRPs) and provide
pharmacist interventions (PIs) to physicians for improving patient care.
However, MR is conducted infrequently by clinical pharmacists in Vietnam because of a lack
of supporting tools.

The level of concordance between users and user-friendliness can be considered as
satisfactory, allowing the use of the tool in daily clinical pharmacy practice. The
present instrument is the first one which was validated for supporting medication review
in Vietnamese hospital.
