Our technical systems are getting more complicated by the day. Whether it’s due to intention or accident this complexity has the same effect on our ability to manage the applications our clients depend on: it gets a lot harder. When the system produces a ‘surprise’ and no longer performs according to assumption effective incident response is critical. Engineers involved must quickly align behind a common goal, communicate efficiently, and predictably coordinate actions to return system behavior to normal. The high level of cohesion necessary to act in this manner doesn’t happen overnight and relying on live-incidents to build this expertise can be painful and costly. In this talk we’ll cover how teams can prepare themselves for the worst of incidents by covering: * The critical building blocks of teamwork that are necessary to bring surprises to resolution; * How to incorporate deliberate practice into the workday to build up incident response muscle memory; and * The incorporation of Chaos engineering practices such as GameDays to realistically simulate how the team will react to a real surprise.

These days we're all trying to find balance in how we use, but not abuse, apps and devices in our day-to-day lives. Not an easy task. It's important to keep aware of all the evidence that's emerging about the impact our digital habits are having on our health. Our team put together this list of potential health hazards associated with tech habits like hopping in and out of 7.9 different communication apps or falling into the "busy trap." Chapter 4 Case study instructions 1. Answer the Case Study Questions (found at the end of each case study) in 500-750 words total (not including reference list). 2. Your case study will be graded on the following: Grading: 20 points Content 80% (how thoroughly and logically you answer the questions, how well you incorporate your reference(s), how well you make arguments and state facts to support your answers). Spelling/Grammar/Punctuation 20% Interactive Session: Organizations Are We Relying Too Much on Computers to Think for Us? Does our ever–burgeoning dependence on computers foster complacency, suppressing our ability to marshal our mental faculties when required? Although computerization has undoubtedly mitigated malfunctions, work stoppages, and breakdowns, are we concurrently losing our ability to assess alternatives independently and make optimal choices? At least one technology writer is sure this is exactly what is happening. Nicholas Carr’s book, The Glass Cage: Automation and Us, lays out the case that our overreliance on computers has dulled our reflexes and eroded expertise. Two cognitive failures undermine performance. Complacency—overconfidence in the computer’s ability—causes our attention to wander. Bias—overconfidence in the accuracy of the data we are receiving from the computer—causes us to disregard outside data sources, including conflicting sensory stimuli. When pilots, soldiers, doctors, or even factory managers lose focus and lack situational awareness, they ignore both suspect data coming from the computer and the external cues that would refute it. The results can be catastrophic. In two instances in 2009, commercial airplane pilots misinterpreted the signals when their autopilot controls disconnected after receiving warnings that the aircraft would stall. Rather than pushing the yoke forward to gain velocity, both pilots heeded faulty control panel data while ignoring environmental cues and pulled back on the yoke, lifting the plane’s nose and decreasing airspeed—the exact opposite of what was required. Loss of automation triggered confusion and panic. Sharply curtailed hands-on flight experience (on a typical passenger flight today, a human pilot mans the controls for just three minutes) resulted in stalled aircraft plunging to earth. Fifty died in Buffalo, New York; 228 perished in the Atlantic Ocean en route to Paris from Rio de Janeiro. The Federal Aviation Administration (FAA) is now pressing airlines to adopt stricter requirements for manual flying hours to offset the risks posed by complacency and bias. Carr’s critics point out that air travel is now safer than ever, with accidents and deaths steadily declining over decades and fatal airline crashes exceedingly rare. Dr. Padma’s Telestroke paper shows how a simple Whatsapp network links doctors and radiologists to create a STROKE network and provide fast stroke care all over a state in India. The HRIDAY CARD is another such innovation. A unique combination of patient education, doctor checklists for heart failure and patient sickness response systems and monitoring charts combined in a single card and already being used in many hospitals in Delhi. HOME-N is a very simple but effective nurse lead heart failure program designed to use all available resources, including the HRIDAY card, smartphone-based apps, and simple telephone calls to improve the outcomes of acute heart failure. In our curriculum section, we take up a case of acute heart failure and discuss the management from myocarditis to assist device implantation. The statistical section discusses the intricacies of the Bland–Altman plot and the history section takes us back to the glamour of the discovery of the various stents. Our readers have sent us a book review of a very interesting book “SAPIENS,” which seems to overturn many of our beliefs and is a must read. Dr. Sarvesh presents his experience with the AIIMS experience with Heart Ware HVAD devices which should be useful to anyone implanting similar devices. Entering our 3rd year of publication, we are gradually increasing our readership and circulation. Our mandate is education. We invite everyone to send articles on themes which provide learning experiences to others. Original articles and cases, journal clubs and bedside cases, and simple journal clubs, etc., are welcome. Edmondson FROM THE APRIL 2011 ISSUE The wisdom of learning from failure is incontrovertible. Yet organizations that do itwell are extraordinarily rare. This gap is not due to a lack of commitment tolearning. Managers in the vast majority of enterprises that I have studied over the past 20 years—pharmaceutical, financial services, product design, telecommunications, and construction companies; hospitals; and NASA’s space shuttle program, among others— genuinely wanted to help their organizations learn from failures to improve future performance. In some cases they and their teams had devoted many hours to after-action reviews, postmortems, and the like. But time after time I saw that these painstaking efforts led to no real change. They also believe that learning from it is pretty straightforward: Ask people to reflect on what they did wrong and exhort them to avoid similar mistakes in the future—or, better yet, assign a team to review and write a report on what happened and then distribute it throughout the organization. These widely held beliefs are misguided. First, failure is not always bad. In organizational life it is sometimes bad, sometimes inevitable, and sometimes even good. Second, learning from organizational failures is anything but straightforward. The attitudes and activities required to effectively detect and analyze failures are in short supply in most companies, and the need for context-specific learning strategies is underappreciated. Organizations need new and better ways to go beyond lessons that are superficial (“Procedures weren’t followed”) or self-serving (“The market just wasn’t ready for our great new product”). That means jettisoning old cultural beliefs and stereotypical notions of success and embracing failure’s lessons. Leaders can begin by understanding how the blame game gets in the way. This new version of the popular embedded device comes with a new iteration of Broadcom’s VideoCore GPU platform, and was released with a fully open source driver stack, developed by Igalia. The presentation explores the development and application of artificial intelligence (AI) from its inception to its current status in the modern world. The term "artificial intelligence" was first coined by John McCarthy in 1956 to describe efforts to develop computer programs capable of performing tasks that typically require human intelligence. This concept was first introduced at a conference held at Dartmouth College, where programs demonstrated capabilities such as playing chess, proving theorems, and interpreting texts. In the early stages, Alan Turing contributed to the field by defining intelligence as the ability of a being to respond to certain questions intelligently, proposing what is now known as the Turing Test to evaluate the presence of intelligent behavior in machines. As the decades progressed, AI evolved significantly. The 1980s focused on machine learning, teaching computers to learn from data, leading to the development of models that could improve their performance based on their experiences. The 1990s and 2000s saw further advances in algorithms and computational power, which allowed for more sophisticated data analysis techniques, including data mining. By the 2010s, the proliferation of big data and the refinement of deep learning techniques enabled AI to become mainstream. Notable milestones included the success of Google's AlphaGo and advancements in autonomous vehicles by companies like Tesla and Waymo. A major theme of the presentation is the application of generative AI, which has been used for tasks such as natural language text generation, translation, and question answering. Generative AI uses large datasets to train models that can then produce new, coherent pieces of text or other media. The presentation also discusses the ethical implications and the need for regulation in AI, highlighting issues such as privacy, bias, and the potential for misuse. These concerns have prompted calls for comprehensive regulations to ensure the safe and equitable use of AI technologies. Artificial intelligence has also played a significant role in healthcare, particularly highlighted during the COVID-19 pandemic, where it was used in drug discovery, vaccine development, and analyzing the spread of the virus. The capabilities of AI in healthcare are vast, ranging from medical diagnostics to personalized medicine, demonstrating the technology's potential to revolutionize fields beyond just technical or consumer applications. In conclusion, AI continues to be a rapidly evolving field with significant implications for various aspects of society. The development from theoretical concepts to real-world applications illustrates both the potential benefits and the challenges that come with integrating advanced technologies into everyday life. In their presentation, O’Brien-Scott and Cakici focused on what Enterprise AI is, why it is important, and what it takes to empower organizations to get started on a search-based AI journey and stay on track.
