DEFINITION OF SHARED CARE
v Share care is a model of health care delivery that involves collaboration between
different healthcare providers in the management of a patient care.
v Share care aims to improve patient outcomes by providing coordinates and
integrated care that is tailored to the patient individual needs.
HISTORY;
It was originated In the United States kingdoms in the 1980s as a respond to
the increasing demand for healthcare services, that is needs to improve the quality
of care provided to the patient.
The termed share care was coined to provide a patient care by contributing
expertise to implement a care plan that meet a patient’s need.
Development of shared care;
The shared Care Model has been widely adopted in the healthcare system around
the world.

Several factor including;
1. Recognition of the importance of coordination and collaboration in healthcare;
Collaboration between healthcare providers by recognizing the
care of a patient’s needs.
2. Advances in technology;
Electronic health records, telehealth, etc.. need easier for the
health care provider to share information and collaborate on the patient care.
3. Changes in healthcare delivery model;
Changes in healthcare delivery models by improving the
healthcare collaboration with another health care provider in the healthcare
systems can help in improving the health of the patient.
4. The rise of chronic diseases management;
It becomes an essential component as patient with chronic
condition required ongoing care for multiple health care providers.

1) Communication and information sharing:
Effective communication and information sharing between healthcare
providers is essential for successful shared care.
2) Fragmented care:
Fragmented care is where the patient may receive conflict advice from
different healthcare providers or may experience gaps in care due to the
lack of coordination between providers.
3) Patient engagement and adherence:
Patient may have difficult engaging with multiple healthcare providers or
adhering to complex care plans, which can lead to poorer health outcomes.
4) Resource allocation:
Implementing shared care need time and funding which may be challenging
to allocate.

It will be difficult to assign accountability to I dividual provider and to ensure that
is fulfilling their responsibilities to the patient.
6) Legal and ethical issues:
The legal and ethical issued has been rise related to patient consent,
confidentiality, and liability that must be addressed to ensure patient safety and
privacy.
IMPACT OF SHARED CARE ON HEALTH CARE SYSTEM:
It has a significance impact on health care system by improving the quality of
care, increasing process to specialize care and reducing healthcare costs.

ASSESS TO SPECIALIZED CARE:
Share care can provide patients with SS to specialize care that may not be
available through their primary care physician alone. This can reduce the burden
on species and improve patient outcome.
HEALTHCARE COST:
Shared care reduce hospital admission and emergency department visit as well
as avoiding duplication of tests and procedure tests can help to improve cost
effective of the health care system.
EFFECIENCY:
It improved the efficiency of healthcare system by reducing administrative tasks
and streamlining communications between healthcare providers.

SHARED CARE MODEL:
The shared care model is a health care delivery model that involves multiple
healthcare providers working together to provide coordinated care to a patient

develop and implement a care plan that meet the patient need.

A healthcare provider worked together with the patient to develop and implement
a health care plan that is tailored to the patient’s need and preference.
CONTINUOUS MONITORING AND EVALUATION:
Healthcare provider continuously monitored patient health and evaluate
effectiveness of the care plan, making adjustment as needed to ensure that the
patient receive the best possible care.

HISTORY; It was originated In the United States kingdoms in the 1980s as a respond to the increasing demand for healthcare services, that is needs to improve the quality of care provided to the patient. The termed share care was coined to provide a patient care by contributing expertise to implement a care plan that meet a patient’s need. Development of shared care; The shared Care Model has been widely adopted in the healthcare system around the world.

Recognition of the importance of coordination and collaboration in healthcare; Collaboration between healthcare providers by recognizing the care of a patient’s needs. 2. Advances in technology; Electronic health records, telehealth, etc.. need easier for the health care provider to share information and collaborate on the patient care. 3. Changes in healthcare delivery model; Changes in healthcare delivery models by improving the healthcare collaboration with another health care provider in the healthcare systems can help in improving the health of the patient. 4. Cont.. 1) Communication and information sharing: Effective communication and information sharing between healthcare providers is essential for successful shared care. 2) Fragmented care: Fragmented care is where the patient may receive conflict advice from different healthcare providers or may experience gaps in care due to the lack of coordination between providers. 3) Patient engagement and adherence: Patient may have difficult engaging with multiple healthcare providers or adhering to complex care plans, which can lead to poorer health outcomes. 4) Resource allocation: Implementing shared care need time and funding which may be challenging to allocate. 8/5/2023 © R R INSTITUTIONS , BANGALORE 6

  • 9. QUALITY OF CARE: Share care promote coordination and collaboration between health care providers, leaving to more efficient and effective care that is tailored to the patient needs. This can result in the better health outcome and improve patient satisfaction. ASSESS TO SPECIALIZED CARE: Share care can provide patients with SS to specialize care that may not be available through their primary care physician alone. This can reduce the burden on species and improve patient outcome. HEALTHCARE COST: Shared care reduce hospital admission and emergency department visit as well as avoiding duplication of tests and procedure tests can help to improve cost effective of the health care system. COLLABORATIVE DECISION-MAKING: A healthcare provider worked together with the patient to develop and implement a health care plan that is tailored to the patient’s need and preference.
