An approved code of practice is a practical guide to achieving the standards of health, safety and welfare
required under the WHS Act and the Work Health and Safety Regulations (the WHS Regulations). A code of
practice applies to anyone who has a duty of care in the circumstances described in the code. In most cases,
following an approved code of practice would achieve compliance with the health and safety duties in the
WHS Act, in relation to the subject matter of the code. Like regulations, codes of practice deal with
particular issues and do not cover all hazards or risks that may arise. The health and safety duties require
duty holders to consider all risks associated with work, not only those for which regulations and codes of
practice exist. Codes of practice are admissible in court proceedings under the WHS Act and Regulations.
Courts may regard a code of practice as evidence of what is known about a hazard, risk or control and may

Right now, I think my current strengths are exercising regularly and being willing to make a change in my
life. My Body Mass Index is also in a healthy range. As for my weaknesses, I have a lot to work on. I know I
need to start eating a healthier diet. I also must work on better stress management, especially because I am
both in school and have a part time job. I could already tell in my day to day life that I needed better sleep
habits as well. The LifeStyle Management Health Risk Inventory has given me some suggestions on how to

Attached below is my personal result of the health risk assessment tool.
My overall heart score was 7.9 out of ten indicating, I do have room for improvement. After reviewing my
scores in different sections of the health risk assessment tool, I realize that I need to increase my level of
physical activity. Currently, I am getting 60 minutes a week of physical activity. The American Heart
Association recommends I incorporate 150 minutes of physical activity into my week. Also, I discovered that
I need to improve my diet by increasing my fruit and vegetable intake. The HRA tool suggested that I
consume five cups of fruits and vegetables a day in addition to 3.5 ounces or more of grains a day (American
Heart Association, n.d.). I am pregnant at this time so I recognize the need to improve my diet. Also, my
blood pressure findings are slightly elevated. This tool also suggested interventions to assist me with
decreasing my blood pressure findings. I appreciate the recommendations the American Heart Association

Health Risks From The Coal Ash Spill
Ultimately, Dr. Shea found very little actual health risks from the coal ash spill. According to Dan Way, Dr.
Shea reported that short–term toxic harm resulted from the spill, and that there were only isolated cases of
coal ash contaminating groundwater (2014). However, there was agreement that long–term monitoring was
necessary at the Dan River to assess longer term damages.
Additionally, the Environmental Protection Agency (EPA) published a response update to assess potential
impacts to human and aquatic life (2014). The method the EPA employed was to analyze surface water and
sediments for contaminants and compare those results to its screening levels. To assess impact to human
health, the EPA compared the sediment data to health risk screening levels and compared drinking water
samples to Maximum Contaminant Levels (MCLs) as well as to other indicators. Overall, the EPA found
those levels found in sediment data and water samples to not exceed human health screening levels and thus
determined that there wasn't significant risk to human health, although the EPA did recommend that people
do not come in direct contact with coal ash as it could still pose health risks. These findings were consistent
with that of Dr. Shea.
Secondarily, the EPA compared the sediment data and water samples to ecological risk based screening
levels. In this study, the EPA did find some levels to exceed ecological screening levels in both the sediment

In the context of risk management, there are several ways that Failure Mode and Effects Analysis can be
used to improve processes in health care organizations. According to Stanley Davis and colleagues,
documenting and analyzing potential risks proactively is essential for improving patient safety (Davis, et al.,
n.d.). The article states that Failure Mode and Effects Analysis (FMEA) is used prospectively to identify the
possible system failures, and to fix the problems to make the system more robust before an adverse event
actually occurs (Davis, et al., n.d.). In a study conducted by G Bonfant and colleagues (2010), FMEA was
used for chronic hemodialysis outpatients. The authors recorded phases and activities, listed activity related
failure modes and effects, described control measures, assigned severity, occurrence and detection scores for
each failure mode and calculated the risk priority numbers (RPNs) by multiplying the 3 scores (Bonfant, et
al., 2010). The authors (2010) also analyzed failure mode causes, made recommendations, and planned new
control measures. Their results showed that the failure modes with the highest RPN was from
communication, and organizing problems (Bonfant, et al., 2010). They (2010) created two tools to fix the
communication flow, including dialysis agenda software, and nursing datasheets. In addition, the authors
scheduled nephrological examinations, and changed medical and nursing organization, and this resulted in a
decrease in RPN value

developing health–related issues within the elderly population. The SPICES evidence–based tool provides a
framework for analyzing older adults concentrating on six comparable "marker conditions": sleep disorders,
problems with eating and feeding, incontinence, confusion, evidence of falls, and skin breakdown (The
Hartford Institute for Geriatric Nursing, n.d.). These circumstances provide a glimpse of a patient 's total
health as well as including the quality of care. The SPICES assessment, done frequently, can indicate the
need for further precise assessment and undergo prevention and treatment of these above–mentioned
conditions. Prior to developing the questions for the interview we looked into some gerontology–related
