REVIEW YOUR OMPF - PAY ATTENTION TO DETAIL! Look for:
•DOCUMENTS THAT ARE FULLY OR PARTIALLY UNREADABLE - make sure
the entire document can be read! If you are having trouble reading it, think about
the selection board member who's read 400 service records today before getting
to yours!
•DOCUMENTS THAT PERTAIN TO SOMEONE ELSE - (Is that someone with the
same name but a different SSN?)
•DUPLICATE DOCUMENTS.
•DOCUMENTS THAT CONTAIN ERRORS.
•MISSING DOCUMENTS like evaluations, page 4’s or 5’s, Personal Awards (Unit
citations and service medals such as the MOVSM are listed on your page 4). The
certificates for these types of awards are not retainable documents.
Try to look at your OMPF with a stranger's eyes - if you need to explain
something as an error, then you should correct it!

DOCUMENTS THAT ARE FULLY OR PARTIALLY UNREADABLE
-To resolve image problems, submit appropriate document(s) as an
enclosure to a letter of explanation to PERS-313.
-Upon receipt of subject document(s), Navy Personnel Command
will delete corrupted images and replace with images of the
document(s) you provide.

ERRONEOUS OR DUPLICATE DOCUMENTS-
-If the document is an Eval/FitRep or relates to an Eval/FitRep you
must contact PERS-32 .
-All others submit an email to PERS-313 to request administrative
corrections to your service record.
-Ensure that you provide the Document Name and Number, Digital
Document Number, Field Code Number, and Page Number (the PDF
page number), and a brief description of the problem.
Example:
Please remove the following document from my record:
1070/604, Enlisted Qual Hist, FC36, DigDoc 12345678 page 2, it
belongs to another member.

-The average turn-around time is 30 days for a request to be
reviewed and acted upon. However, it may exceed 30 days during
upcoming Boards due to the higher volume of requests.
-Please do not submit more than one request.
PERS-313 only manages your image (OMPF) record.
Your online record is corporate data and will need to be addressed
through your local administration for correct procedures. See your
PSD or local admin.

The CNO Awards Office established and maintains the NDAWS as the single
authoritative data base for all Military Decorations. All awards entered into the
NDAWS data base are exported to the BUPERS Mainframe for updating
pertinent personnel records. To ensure accurate, complete, and
synchronized awards data between NDAWS and the personnel records,
awards data should only be entered into NDAWS. To query awards, go to:

-Third party correspondence will not be permitted.
-All correspondence should be on plain white paper, paper
clipped (no staples, binders, folders, or tabs), and submitted
under cover letter to the president of the board.
-Candidates must verify the correct board number (see below) is
on their cover letter to ensure their letter appears before the
proper board.
-Full name and full social security number must be affixed and
legible on all documents submitted.

Sailors are welcome to provide new information to the board that is
not currently in their record. Old or redundant information could
frustrate record reviewers who must review all correspondence sent.
Letters to the board may be mailed or e-mailed to the addresses
above, providing candidates the opportunity to submit any missing
documents from the sections of their OMPF that are reviewed by the
board. The E7 selection board will review the OMPF of all candidates,
as well as items submitted by candidates per policy outlined in
NAVADMIN 349/11
Each eligible candidate is responsible for ensuring their OMPF is
correct and up to date with latest evaluations, awards, and other
appropriate information.
